Job summary

CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to join the team, reporting to the Clinical Manager. The role involves administering PT treatment in patients' homes and coordinating with the PT and office staff to ensure timely documentation.

Requirements include an associate degree in Physical Therapy, current state licensure, CPR, and a year of PTA experience. A valid driver's license and reliable transportation are also required.

Qualifications

• Associate degree in Physical Therapy from an American Physical Therapy Association (APTA) program.
• Current state license to practice Physical Therapy.
• CPR certification is current.
• One year of experience as a Physical Therapy Assistant is preferred.
• Fluent English read/write/speak.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Perform non-complex manual therapy techniques and modalities as prescribed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
