Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team serving Cleveland/Splendora, TX. The PT will evaluate patients at home, develop treatment plans, and deliver evidence-based interventions to improve mobility and reduce pain.

Ideal candidates hold a TX license, have home health or rehab experience, and can educate patients and caregivers. Excellent communication and adaptability are essential; flexible scheduling and a supportive team environment are offered.

Qualifications

• Active Physical Therapist license in Texas (TX).
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Professional liability insurance coverage for TX license.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatments, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall-prevention measures.
• Deliver compassionate care and promote independence.
