PARS Therapy is seeking a compassionate and skilled Physical Therapist (PT) to join our dedicated home health team in Sealy, TX. This role focuses on helping patients regain movement, reduce pain, and improve quality of life in the comfort of their own homes.

The ideal candidate is patient-centered, dependable, and committed to delivering evidence-based care with excellent communication to patients and caregivers, while collaborating with an interdisciplinary team.

Qualifications

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage for TX PT license.
• Graduate degree or higher in Physical Therapy.
• Experience in home health or rehabilitative settings preferred.
• Adapt care approaches to diverse patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication skills with patients and caregivers.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor patient progress and adjust plans as necessary.
• Document evaluations, treatments, and progress notes using digital platforms.
• Collaborate with interdisciplinary team members (physicians, nurses, OTs).
• Conduct home safety assessments and make fall-prevention recommendations.
• Deliver compassionate care while promoting independence.
