Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to provide home-based care. You will assess and treat patients to improve mobility and daily living skills, following physician orders and the established care plan.

The role requires state licensure, a valid driver's license, and reliable transportation. You will collaborate with families and the care team to optimize outcomes and ensure compliance with the plan of care.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must be currently licensed as a Physical Therapist in the state of employment.

Responsibilities

• Examine and treat patients to improve mobility and function.
• Administer skilled care under physician orders and care plan.
• Educate family and health care team on treatments and caregiver support.
• Follow the plan of care under Branch Director supervision.
