You will assess and evaluate patient care needs related to functional status, ADLs, fine motor skills, and home assessments, and support adaptive equipment decisions as defined by medical conditions.

Qualifications

• Current Occupational Therapy licensure in state of practice
• Current driver’s license and vehicle insurance or access to dependable transportation
• Preferred CPR certification or ability to obtain within 90 days of employment
• Demonstrate ability to manage multiple tasks simultaneously
• Demonstrate ability to work independently
• Good communication, writing, and organizational skills

Responsabilités

• Evaluates the patient’s functional status and CMS documentation readiness; consults with physician for therapy plan of care
• Observes, records, and reports patient response to treatment and condition changes
• Conducts patient assessments using functional ability tests
• Instructs patient, family, caregiver, and health care team in therapy activities
• Functions effectively in home settings regardless of age, race, creed, color, sex, disability, or financial condition
