Job summary

Baptist Home Care, part of LHC Group and the Optum family, seeks a Per Diem Occupational Therapist to assess functional status, ADLs, fine motor skills, and home adaptations for patients in the community.

Responsibilities include evaluating function, documenting CMS data, guiding patients and families, and collaborating with nurses and physicians to implement the therapy plan. A current OT license in the practicing state and reliable transportation are required; CPR certification is preferred.

Qualifications

• Must hold current OT licensure in the state of practice.
• Valid driver's license or access to reliable transportation.
• CPR certification or ability to obtain within 90 days (preferred).
• Ability to manage multiple tasks concurrently.

Responsibilities

• Evaluates the patient's functional status and develops the plan of care with physician input.
• Observes, records, and reports patient responses and changes to the care team.
• Conducts patient assessments evaluating functional ability using standard tests.
• Instructs patient, family, caregiver, and care team in therapy techniques to maximize participation.
• Provide care in home settings regardless of age, race, creed, color, sex, or disability.

As the Per Diem Occupational Therapist, you will be responsible for assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.

Primary Responsibilities:

• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests
• Instructs patient, family, caregiver, and participating members of the health care team in the areas of therapy in which they can participate to assist the patient
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:

• Current Occupational Therapy licensure in state of practice
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation

• Current CPR certification or ability to obtain with 90 days of employment
• Demonstrate ability to manage multiple tasks simultaneously
