Baptist Home Care, part of the LHC Group and Optum family, seeks a Per Diem Occupational Therapist to assess patients' functional status, daily living activities, motor skills, home safety, and adaptive equipment needs.

You will collaborate with physicians, nurses, families, and caregivers to develop therapy plans, document CMS-relevant findings, and guide home-based care across diverse patient settings.

Qualifikationen

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to dependable transportation.
• CPR certification preferred or obtainable within 90 days.

As the Per Diem Occupational Therapist, you will be responsible for assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.

Primary Responsibilities:

• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests
• Instructs patient, family, caregiver, and participating members of the health care team in the areas of therapy in which they can participate to assist the patient
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:
