Job summary

CenterWell Home Health is seeking a dedicated Home Health Aide (HHA) to support patients in regaining strength, mobility and independence at home. You’ll work under an RN and follow the Aide Plan of Care to deliver rehabilitation-focused care, helping patients perform daily activities with confidence and improved quality of life.

The role requires CNA/HHA certification within 24 months, state certification, a valid driver’s license, and at least one year of relevant experience.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver's license, auto insurance, and reliable transportation are required.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist the patient with self-administered medications and observe intake.
• Take accurate temperature, pulse and blood pressure measurements and report abnormal results.
• Help patient maintain personal hygiene and assist with bathing, grooming, and oral care.
• Assist in feeding patients and support self-feeding; provide nutrition education.
• Assist with patient toileting and ostomy care as needed.
• Support mobility, exercises, transfers and ambulation per Plan of Care.
• Document care rendered and communicate with caregivers, doctors and staff.
• Maintain infection control and provide safe, clean environment.
• Assist with light household tasks and errands when allowed.
