CenterWell Home Health is seeking a dedicated Home Health Aide to assist patients in their homes under RN supervision. You will follow the Plan of Care, help with medications, monitor vital signs, and support daily living activities to promote independence and quality of life.

Requires a recent CNA/HHA program completion, valid MA driver’s license, and state certification. Prior home health or hospital experience preferred. Competitive benefits and a focus on safe, patient-centered care.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• Must have valid driver’s license, auto insurance, and reliable transportation.
• Must be in good standing on the HHA Registry and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience as a Home Health Aide or CNA in clinical settings within last 24 months.

Responsabilités

• Provide direct patient care under RN direction per the Plan of Care.
• Assist with self-administered medications as directed and monitor adherence.
• Take and report temperature, pulse, and BP; identify abnormal results.
• Assist with bathing, grooming, hygiene and other activities of daily living.
• Assist with feeding and manage nutrition and hydration; support self-feeding as needed.
• Assist with toileting, ostomy care, and continence management.
• Support mobility exercises, transfers, and ambulation per Plan of Care.
• Document care provided and communicate changes to team.
• Maintain infection control and provide a safe home environment.
• Assist with light household tasks if required and permitted.
