Job summary

CenterWell Home Health is seeking a dedicated Home Health Aide to deliver personalized care in patients0homes. The role supports rehabilitation and independence, under RN supervision, with duties spanning daily living tasks, vitals, medication assistance, and safe mobility.

Qualified candidates have a current CNA/CHHA credential, valid driver’s license, and state certification. At least one year of home health or CNA experience within the last 24 months is preferred.

Qualifications

• Must meet applicable state certification requirements.
• Must have a valid driver’s license, auto insurance, and reliable transportation.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency within the last 24 months.

Responsibilities

• Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist with self-administered medications by opening bottle caps, reading labels, and observing administration.
• Take vital signs and report abnormal results to supervisor.
• Assist with personal hygiene, bathing, grooming and oral care.
• Assist with feeding and monitoring nutrition and fluid intake.
• Assist with toileting and catheter care as ordered.
• Assist with mobility, positioning, transfers and ambulation per Plan of Care.
• Document care provided and changes in patient condition for continuity of care.
• Maintain infection control, provide a clean and safe environment.
