Job summary

Clearbrook is seeking a Habilitation Aide at the Commons in Rolling Meadows, IL to support individuals with disabilities across daily living, self-care, and community activities. You will assist with training and implement person-centered plans under nursing coordination.

Hours include AM/PM and weekends, with part-time or full-time options. The role offers paid training and a benefits package featuring health, dental, life and disability insurance, a 403B plan, paid time off, and employee

Qualifications

• 8th grade diploma or equivalent is required.
• Valid IL driver’s license required.
• Must be 16 years of age.

Responsibilities

• Responsible for the direct provision of active treatment to the clients including training and assistance in all areas addressed in the individual program plans of the clients to whom she/he is assigned.
• Teach and provide the support necessary for the individual’s use of community resources.
• Assist clients with daily living activities including (but not limited to) personal hygiene, feeding & housekeeping.
• Assist clients in all areas of social and recreational program activities for the day.
