Clearbrook is seeking a Habilitation Aide–ICF in Rolling Meadows, IL for a full-time night shift (Mon-Thu 11:00pm-9:00am). Hourly pay is $21.35 with training, weekend differential, and a medication pass differential.

Responsibilities include direct client care, teaching use of community resources, and assisting with daily living activities. Qualifications include a high school diploma, a valid IL driver’s license, and being at least 21 years old; experience with behavior disorders and autism is

• Previous experience with behavior disorders and autism-related issues preferred.

Responsibilities

• Provide direct active treatment to clients as outlined in individual program plans.
• Teach and support use of community resources.
• Assist clients with daily living activities (hygiene, meals, housekeeping).
• Assist with social and recreational activities for the day.
• Must be able to perform a 2-person lift.
• Staff must be awake for the full shift.

Habilitation Aide–Wilke ICF

Clearbrook has threeIntermediate Care Facilities (aka ICF’s), each housing multiple clients. Goals are developed for each client based on their needs. Programming is assigned, usually pertaining to areas such as self-care, along with community living & integration. Each facility has a nurse on staff at least 20 hours per week to coordinate any medical needs required by the clients.

Location: Rolling Meadows, IL

Hours: Full-Time

Schedule: Monday-Thursday: 11:00pm-9:00am (40 hours/wk)

Pay: $21.35/hour +PaidTraining!

Med Passer Differential: + $1/hr during medication pass periods

Weekend Pay: + $3/hr differential for weekends

Responsibilities:

• Responsible for the direct provision of active treatment to the clients including training and assistance in all areas addressed in the individual program plans of the clients to whom she/he is assigned.
• Teach and provide the support necessary for the individual’s use of community resources.
• Assist clients with daily living activities including (but not limited to) personal hygiene, feeding & housekeeping.
• Assist clients in all areas of social and recreational program activities for the day.
• Must be able to perform a 2-person lift
• This is not a sleep facility - staff are required to be awake for the duration of entireshift

Qualifications: Requirements:

• High Schooldiploma or equivalent
• Valid IL driver’s license required
• Must be 21years of age
