Job summary

Clearbrook is seeking a Habilitation Aide for the ICF Fairfax program in Rolling Meadows, IL. The role provides direct active treatment, supports daily living tasks, and helps clients participate in community activities.

You will work part-time with a schedule including weekdays and weekend hours. The position offers paid training, weekend differential, and opportunities to contribute to person-centered supports for individuals with disabilities.

Qualifications

• High School diploma or equivalent.
• Valid IL driver's license required.
• Must be 21 years of age.

Responsibilities

• Direct provision of active treatment to clients according to individual program plans.
• Teach and support use of community resources.
• Assist clients with daily living activities including personal hygiene, feeding, and housekeeping.
• Assist clients in all areas of social and recreational program activities for the day.

Schedule: Wednesday 6am-10am & 2:30pm-11pm, Sunday 9am-9pm (24 hrs)

Pay: $21.35/hour + Paid Training!

Med Passer Differential: +$1/hr during medication pass periods

Weekend Pay: +$3/hr differential

Clearbrook has three Intermediate Care Facilities (ICFs), each housing multiple clients. Goals are developed for each client based on their needs. Programming is assigned, usually pertaining to areas such as self-care, along with community living & integration. Each facility has a nurse on staff at least 20 hours per week to coordinate any medical needs required by the clients.

Responsibilities

• Responsible for the direct provision of active treatment to the clients including training and assistance in all areas addressed in the individual program plans of the clients to whom she/he is assigned.
• Teach and provide the support necessary for the individual’s use of community resources.
• Assist clients with daily living activities including (but not limited to) personal hygiene, feeding & housekeeping.
• Assist clients in all areas of social and recreational program activities for the day.
