Job summary

AHRC Nassau is seeking dedicated Direct Support Professionals to assist individuals with intellectual and developmental disabilities in daily living, health, safety, and community participation. The role includes personal care, meal support, transportation, and collaboration with an interdisciplinary team to promote independence.

Applicants should have a High School Diploma or equivalent and a valid NYS driver’s license.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license, and maintain a qualified driver status throughout employment.
• Must have the ability to communicate and understand written and oral directives/information.

Responsibilities

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in every aspect of their lives.
• Provide support to become active members of the community and maintain/build natural supports and relationships.
• Ensure a safe and clean home by assisting with household chores and participating as needed.
• Follow treatment plans and use adaptive/safety equipment to promote independence and well-being.
• Assist individuals with meal preparation including diet adherence and portion control.
• Aid in medication administration according to AMAP policy.
• Document treatment plans and related materials to support independence and health.
• Conform to overtime, scheduling, and staffing policies to maintain daily quality support.
• Attend interdisciplinary team meetings and share progress and ideas.
• Provide transportation for appointments, shopping, and outings.
• Mentor new Direct Support Staff and review training materials.
