Job summary

AHRC Nassau seeks a Direct Support Professional to provide ongoing guidance and support to individuals in the Community Living Services program, promoting independence and a high quality of life. You will assist with self-care, health, safety, meals, transportation, and daily routines, in collaboration with an interdisciplinary team.

Qualifications

• Strong communication skills and ability to understand written and oral directives.
• Must submit a valid NYS driver's license and maintain it throughout employment.
• Ability to read and follow instructions and work with individuals with disabilities.

Responsibilities

• Provide ongoing guidance and support to individuals to ensure active participation in daily life.
• Support individuals to become active community members and maintain relationships.
• Ensure a safe and clean home by assisting with household chores and participating as needed.
• Follow treatment plans and use adaptive equipment to promote health, safety, and independence.
• Assist with meal preparation, diet adherence, and cooking skills.
• Assist with medication administration per AMAP policy and procedures.
• Document treatment plans and progress to help individuals achieve goals.
• Participate in meetings and support the inter-disciplinary team.
• Provide transportation for appointments, shopping, and outings.
• Mentor new Direct Support Staff and review training materials.
• Attend required trainings and maintain certifications.
