• Provide information about community resources and help self-advocate.
• Drive participants to appointments and activities.
• Maintain accurate documentation per guidelines and ensure timely submission.
• Provide crisis intervention as needed and coordinate with support systems.
• Maintain professional relationships with participants, families, co-workers and agencies.
