Qualifications

• Provide information about community resources.
• Drive program participants to appointments.
• Ensure access to appropriate resources for participants.
• Conduct ongoing assessments of program participants.
• Build rapport through effective communication.
• Maintain accurate documentation on program participants.
• Provide timely documentation for program participants.
• Provide crisis intervention as needed.
• Maintain professional relationships with program participants.
