Job summary

AHRC Nassau in Merrick, NY is seeking a Direct Support Staff member to provide comprehensive support to individuals in the Community Living Services program. You will assist with daily routines, health, safety, and personal care, enabling residents to maintain quality of life and independence.

This role requires carrying out treatment plans, supporting meals and transportation to appointments, and collaborating with a multidisciplinary team.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license, and maintain a qualified driver status throughout employment.
• Must have the ability to communicate and understand written and oral directives/information

Responsibilities

• Provides ongoing guidance and support to individuals to ensure active participation in daily life.
• Provide support to become active members of the community and maintain relationships.
• Ensures a safe and clean home by assisting with household chores.
• Follows treatment plans and utilizes adaptive and safety equipment as needed.
• Supports individuals in meal preparation and medication administration per policy.
• Implements and documents treatment plans and other agency materials to promote independence.
• Provides transportation to medical appointments, shopping, and activities.
• Mentors new Direct Support Staff and participates in staff meetings.
• Performs other duties as requested by supervisor.
