Job summary

Seven Counties seeks a dedicated case management professional to assess client needs and coordinate appropriate services. You will participate in creating individualized service plans, arrange required services, and assist clients with referrals to access essential supports.

You will monitor progress, document outcomes, and collaborate with supervisors to ensure high-quality care. The role requires a Bachelor’s degree in Social Work or related field and strong interpersonal and communication

Qualifications

• Good interpersonal skills and written and oral communication skills.
• Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client’s needs.
• Participates in development of client’s service plan, consistent with assessment.
• Arranges for delivery of needed services, as identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client’s progress by making referrals, tracking appointments, and performing follow-ups.
