Job summary

Southend Care is seeking a dedicated Support Worker to assist tenants in living independently and making informed choices. You will work with tenants, families, health professionals and partners to deliver high-quality, person-centred support across services.

Role involves activities to promote dignity, rights and wellbeing, including daily living skills, community access, and coordinated care planning. Commitment to safeguarding and flexibility to work shifts is essential.

Responsibilities

• Build positive relationships with the people we support.
• Help tenants make informed choices and develop independence.
• Assist access to community services and activities.
• Support with daily living skills and personal development.
• Act as a key worker for care plans and reviews.
• Assist with risk assessments and adherence to plans.
• Support budgeting and keeping financial records.
• Help with benefits and paperwork as required.
• Administer or prompt medication per procedures.
• Provide personal care while maintaining dignity and privacy.
• Accompany to appointments, courses and activities.
• Coordinate with families and professionals for holistic care.
• Support health, cultural and spiritual needs and advocacy.
• Maintain a clean, safe venue and secure belongings.
• Keep accurate digital records and provide handovers.
• Report safeguarding concerns promptly.
• Respect confidentiality and boundaries.
• Work as part of a team and support the service lead.
• Undertake training and develop professionally.
• Promote Southend Care values and rights of people we support.
