Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to assist in delivering personalized care to patients at their homes. You will work under the supervision of a Physical Therapist to help patients regain strength and independence through functional improvement.

Responsibilities include implementing treatment plans, performing therapeutic modalities, documenting progress, and coordinating with the care team. A valid PT license, driver's license, and CPR are required.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of the physical, emotional, social, and biological changes in the geriatric population.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Administer non-complex active and passive manual exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
• Apply infection control and universal precautions in patient care activities.
