Job summary

Mental Health Cooperative in Nashville is seeking a Supported Employment and Education Specialist, FEPI to develop and link employment and educational opportunities for FEPI Consumers using CSC and IPS models. The role collaborates with agencies, educational institutions, and care teams to create individualized placement paths.

Responsibilities include establishing employer relationships, assisting Consumers with resumes and job searches, tracking outcomes, and ensuring thorough documentation

Qualifications

• Bachelor’s or Associate degree in education, social work, human resources, healthcare, or disability related field of study; OR High school diploma or GED and two years of experience in related fields.

Responsibilities

• Develop and establish relationships to gain transitional and supported employment positions with agencies.
• Provide consumer care within Coordinated Specialty Care (CSC) model.
• Provide consumer care within IPS model.
• Assist team in identifying consumers’ education and employment goals.
• Accompany consumers to school meetings, job interviews, and related meetings.
• Attend trainings and conferences as required.
• Assist consumers with resume building and job applications.
• Help consumers gain and enhance skills while working with them.
• Monitor, coach, and train consumers in the SE/IPS program.
• Maintain communication with employers for placements and build professional relationships.
• Develop relationships with educational institutions and resources; refer and advocate for individual needs.
• Complete all documentation per Policies and Procedures.
• Attend team meetings and follow up on plans; participate in all-hands meetings/trainings.
• Train, link, and access members interested in employment according to strengths and abilities.
• Track data and statistics related to supported employment.
• Ensure timely documentation as required by the Agency.
