Job summary

Community Living, Inc. seeks a Vocational Case Manager to help individuals with intellectual and developmental disabilities discover meaningful employment opportunities.

You will work one-on-one with participants to identify strengths, connect with local employers, and provide the supports needed to succeed in the workplace. This full-time role may involve meetings with employers, coaching participants through interviews, and coordinating plans while ensuring funding and compliance requirements

Qualifications

• High school diploma or GED required.
• Bachelor's degree in Social Services or related field preferred.
• Experience supporting individuals with intellectual and/or developmental disabilities is preferred.
• Strong communication, organization, and time management skills.
• Comfortable balancing multiple participants and priorities.
• Ability to build positive relationships with employers and community partners.
• Proficiency with Microsoft Office.
• Valid driver's license and reliable transportation.

Responsibilities

• Partner with participants to identify employment goals and career interests.
• Build relationships with local employers and develop job opportunities.
• Conduct assessments to match participants with the right employment.
• Assist participants with job searches, applications, and interviews.
• Provide job coaching and on-site support to help participants succeed.
• Monitor employment progress and adjust supports as needed.
• Complete documentation, case management, and monthly reporting requirements.
• Coordinate individual plans and maintain participant records in Therap.
• Collaborate with families, employers, and interdisciplinary teams.
• Ensure services meet all funding and compliance requirements.
