Under supervision, you will place participants in appropriate work sites within the county, document progress in CMS, and coordinate with DSS and other partners.

Formación

• Education: High School diploma.
• Experience: 1-2 years of case management experience in a human services setting.

Responsabilidades

• Engage individuals in job readiness activities to prepare for future employment.
• Place participants in suitable worksites within the county using referral systems.
• Maintain CMS case notes reflecting changes in case status and outcomes.
• Attend required meetings and trainings and perform other duties as assigned.
