Job summary

MyPath seeks a Float-Direct Support Professional to provide direct care and supervision in Waukesha, WI. You will implement ISPs/ITPs and BSPs and support individuals with daily routines to promote health and safety.

You will assist with activities of daily living, mobility, meals, and household tasks, while promoting skill development, independence, recreation and community engagement, and documenting progress for the care team.

Responsibilities

• Provide direct care and supervision of individuals served.
• Implement ISPs/ITPs, BSPs, and program routines.
• Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.
• Facilitate activities that promote skill development, independence, recreation, and community engagement.
• Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy.
• Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
• Safely transport individuals to appointments, activities, or community outings as required.
