Job summary

MyPath is seeking dedicated direct care staff to support individuals with special needs in Milwaukee. You will provide direct care and supervision, ensure health and safety, and assist with activities of daily living, meals, mobility, and household tasks.

You will implement ISP/ITP plans, BSPs, and program routines to promote skill development and independence. You will monitor progress, document health concerns and incidents, communicate with the care team and supervisors, and transport

Responsibilities

Provide direct care and supervision of individuals served, ensuring their health, safety, and well-being.
Implement Individual Service Plans (ISPs) and Treatment Plans (ITPs), Behavior Support Plans (BSPs), and program routines.
Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.
Facilitate activities that promote skill development, independence, recreation, and community engagement.
Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy.
Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
Safely transport individuals to appointments, activities, or community outings as required.
Maintain a clean, safe, and supportive environment while following all safety and regulatory requirements.
