Qualifications

• Graduate of an approved school of nursing.
• Current Massachusetts LPN license in good standing.
• Previous nursing experience preferred.
• Understand the philosophy and model of care at Hebrew Rehabilitation Center.
• Professional, proactive, collaborative and results-oriented.

Responsibilities

• Ensure care delivered adheres to safe professional nursing practice and established standards.
• Collaborate with MD/NP, nurses, social workers, therapists, and dietitians to meet patient needs.
• Adhere to evidence-based interventions for optimum outcomes.
• Operate within a primary nursing model with RN oversight for assessment and care planning.
• Contribute to care planning and support patient autonomy and choice.
• Ensure care plans reflect patient needs and are followed and revised as needed.
• Document care in EMR and initiate reports (DPH, incident, concerns).
• Identify resources to ensure safe, effective care and coordinate with team.
• Delegate and supervise care delivered by nursing assistants; pursue learning.
