Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Working knowledge of computers.

Responsibilities

• Provide direct support per individual service plans, including socialization, health maintenance, and daily living activities.
• Assist with daily living activities: personal care, housekeeping, transportation, community integration, shopping.
• Document progress and maintain confidentiality of records.
