Job summary

LCMC Health seeks a Substance Use Navigator to identify and support patients with SUDs in the emergency department, linking them to MAT and community services. You will collaborate with ED clinicians, social work, behavioral health, and partners to provide non-judgmental care that promotes recovery-oriented outcomes.

Ideal candidates bring 2+ years in SUD or related fields, with strong interpersonal skills and cultural humility.

Qualifications

• Minimum of two years working with individuals with SUD or mental health conditions in healthcare, community, or social services.
• ED/ inpatient or outreach experience preferred; MAT programs valued.
• Lived recovery experience can count in lieu of formal experience.

Responsibilities

• Identify and engage patients with SUD in the ED.
• Facilitate access to MAT and harm-reduction resources.
• Coordinate care across hospital teams and community providers.
• Assist with transportation, records, and referral follow-ups.
• Support patients through appointments and follow-up communication.
