Job summary

Ochsner Health seeks a Substance Use Navigator to engage ED patients with substance use concerns, perform SBIRT, and link them to treatment and community resources. This role supports rapid referrals and coordination with ED clinicians, social workers, and behavioral health teams, including MAT and recovery services.

The SUN uses trauma-informed care to build trust,may leverage lived experience appropriately, and may work variable shifts to support ED operations.

Qualifications

• High School Diploma or GED required.
• Preferred degrees in Social Services, Behavioral Health, Public Health.
• Experience in healthcare, behavioral health, crisis outreach, or related setting.

Responsibilities

• Engage ED patients with substance use concerns using trauma-informed, person-centered approaches.
• Establish rapport with patients in crisis or ambivalent about treatment.
• Perform SBIRT activities and provide education on options and referrals.
• Coordinate real-time referrals and warm handoffs to MAT, recovery services, and community resources.
• Document all interactions and referrals in the EHR and assist with care coordination.
