Job summary

NALHE is seeking a hospital social worker to join the emergency department and CDU social medicine team. You will perform psychosocial assessments, identify needs, and collaborate on care plans with the care team to support patients and families in crisis.

You will document in patient records, coordinate community referrals, and participate in discharge planning and quality initiatives. Ongoing professional development and cross-team collaboration are essential to success.

Qualifications

• Performs psychosocial assessments and coordinates social medicine services.
• Provides information and referrals to community resources for post-discharge support.
• Participates in discharge planning and ensures appropriate community placement.
• Maintains documentation according to departmental standards and regulations.
• Contributes to performance improvement and quality initiatives within the department.

Responsibilities

• Participates as a member of the ED/CDU care team providing social medicine expertise.
• Completes psychosocial assessments of patients in ED/CDU.
• Identifies psychosocial needs and develops care plans.
• Documents interactions and tasks in medical records in a timely manner.
• Assists patients and families with coping strategies related to illness.
• Provides information, referrals, and advocacy to patients and families.
• Plans for ongoing learning and participates in departmental functions and committees.
