Job summary

Responsibilities

• Provide psychosocial assessments, care planning, intervention, and evaluation.
• Coordinate discharge planning and community resource referrals.
• Provide crisis intervention and advocacy for patients and families.
• Communicate with ED Care Team and other care providers about patient progress.
• Document treatment plans, progress notes, and responses to interventions.
• Support with behavioral health housing placements and CQI activities.
