Qualifications

• Bachelor’s Degree in Social Work required.
• Master’s Degree in Social Work preferred.
• Valid NC Driver’s License required.
• BLS certification required.
• CPI certification required.
• Minimum of two years paid experience in social work.

Responsabilités

• Provide psychosocial assessment, care planning, intervention and evaluation to individuals and families.
• Document plan of care and progress notes.
• Coordinate discharge planning and transitions.
• Address crisis interventions and patient advocacy.
• Collaborate with ED Care Team and community resources to support care needs.
• Communicate effectively with patients, families and healthcare team while maintaining confidentiality.
