Job summary

Qualifications

• Physical requirements: Medium work up to 50 lbs., frequently up to 20 lbs., and constant up to 10 lbs.

Responsibilities

• Assess patient needs across physical, psychosocial, cultural, and spiritual dimensions through observation and chart review.
• Develop and implement patient care plans and monitor outcomes with the interdisciplinary team.
• Perform interventions including IVs, wound care, meds, catheterization, and specimen collection.
• Maintain accurate and timely patient charts and use IT to communicate and support decision-making.
• Advocate for patient health, safety, and rights; educate patients and families and assess teaching effectiveness.
