• RN with active license and BLS certification.
• Minimum 1 year of 12-lead EKG experience.
• Minimum 1 year of acute hospital experience preferred.
• Strong communication and teamwork skills.

Responsibilities

• Conduct comprehensive clinical assessments.
• Develop and implement individualized care plans.
• Coordinate multidisciplinary care with physicians, nurses, social workers, and therapists.
• Monitor patient progress and adjust plans as needed.
