Job Summary:

As the leader of the health care team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. Demonstrates performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization. Remains flexible to changing systems; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats co-workers, patients, families, and all members of the health care team with dignity and respect.

Essential Responsibilities:

  • Nursing Process:
  • Develops and contributes to individualized care plans based on assessment, planning, implementation, and evaluation.
  • Ensures multidisciplinary planning, consultation, and education are reflected in care plans.
  • Adapts care plans based on ongoing patient assessment and progress.
  • Ensures care meets professional standards and standards of practice.
  • Develops and completes discharge plans, addressing barriers and providing patient/family education.
  • Ensures patient understanding of discharge instructions and medication management.
  • Patient Care Experience:
  • Practices customer service standards as defined by the organization.
  • Responds promptly to call lights, alarms, and patient requests.
  • Makes referrals and facilitates resource utilization.
  • Maintains patient confidentiality and a clean, safe environment.
  • Treats patients and families with courtesy, respect, kindness, and compassion.
  • Actively listens to patient needs and ensures those needs are met.
  • Provides clear and understandable information to patients.
  • Strives to exceed patient expectations in care delivery.
