Job Summary:
As the leader of the health care team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. - Nursing Process:
- Develops and/or contributes to individualized care plans based on assessments, planning, implementation, and evaluation of outcomes.
- Ensures multidisciplinary planning, consultation, and education are reflected in the care plan.
- Ensures the care plan considers admission or outpatient data, ongoing findings, age-appropriate care, cultural needs, and acuity.
- Discusses the care plan with patients and families/significant others and completes it in a timely manner.
- Monitors patient progress based on the care plan.
- Revises the care plan as needed based on patient condition and progress evaluation.
- Ensures care meets standards of practice.
- Develops and completes discharge plans, addressing barriers and providing patient/family education.
- Ensures patients understand discharge instructions; identifies barriers and educational needs for outpatient care.
- Prioritizes patient safety, including medication safety, fall prevention, decubitus prevention, and infection control.
- Optimizes pain control and patient comfort; addresses patient anxieties, fears, and concerns.
- Ensures patients understand medication purposes, side effects, and administration instructions both during hospitalization and at discharge.
