POSITION SUMMARY
Arthur Center is looking for a dedicated, resourceful Emergency Room Enhancement (ERE) Care Coordinator to join our Behavioral Health Outreach team and play a vital role in connecting residents to the services and support they need. In this role, you will engage clients who frequently interact with emergency services, coordinate access to mental health, medical, and community resources, and provide the follow-up and coaching that helps people achieve and maintain health and independent functioning. KEY TASKS AND RESPONSIBILITIES
CLIENT ENGAGEMENT AND RESOURCE NAVIGATION
- Engage and support clients who frequently interact with emergency services within the catchment area.
- Provide client assistance in accessing mental health and medical services.
- Assist clients with accessing necessary public services including financial assistance, housing, and basic needs for food, shelter, and clothing.
- Aid clients with accessing social, educational, vocational, health, and housing resources identified as needs for independent functioning.
- Provide training, coaching, and support in daily living skills including housekeeping, cooking, personal grooming, accessing transportation, budgeting, paying bills, and maintaining independent residence.
CARE COORDINATION AND FOLLOW-UP
- Provide adequate follow-up with clients regarding appointments, completion and submission of forms, and other necessary actions.
- Assist with coordination of hospital discharges as needed to ensure smooth transitions of care and prevent readmissions; engage existing providers to ensure continuity of care.
- Establish and maintain positive working relationships with local community resources including law enforcement, ambulance personnel, crisis call centers, and hospitals.
- Attend agency and community meetings as assigned by supervisor.
CRISIS RESPONSE AND TRAUMA-INFORMED PRACTICE
- Assist with daytime mobile crisis response as needed.
- Participate in crisis on-call secondary duties as scheduled.
- Engage with clients in a trauma-informed and collaborative manner that supports safety, empowerment, voice, and choice.
- Respond to community crises in a timely manner and apply CPR and Crisis Prevention Institute (CPI) de-escalation and behavioural management techniques as needed.
DOCUMENTATION AND COMPLIANCE
- Document all client interactions and contacts according to agency and DMH standards.
- Meet all internal and external reporting requirements including data collection for Quality Assurance evaluation and DMH reporting.
- Attend educational seminars and professional development meetings as required.
- Perform other duties as requested by supervisor.
SKILLS AND ATTRIBUTES
- Strong interpersonal engagement skills for adults and seniors across diverse backgrounds and circumstances.
- Demonstrated ability to develop and maintain strong partnerships with law enforcement, ambulance personnel, crisis call centers, crisis centers, and hospitals.
- Highly flexible and adaptable — comfortable responding to a wide range of client presentations and community situations.
- Strong verbal and written communication skills with the ability to maintain accurate, timely documentation.
- Committed to trauma-informed, culturally responsive care and client self-determination.
- Collaborative team player with experience working within a multidisciplinary care model.
REQUIRED QUALIFICATIONS
- Bachelor's degree in social work, nursing, psychology, mental health, human services, or a related field required.
- Master's degree in a related field preferred.
- Experience in behavioral Health, or related field.
- Must be able to respond to community crises in a timely manner and apply CPR and CPI de-escalation techniques as needed.
