Job summary

Arthur Center in Mexico, MO seeks an Emergency Room Enhancement (ERE) Care Coordinator to join our Behavioral Health Outreach team. You will connect residents who frequently interact with emergency services to mental health, medical, and community resources, providing follow-up and coaching for health and independent living.

This is community-based, trauma-informed work that requires strong organizational skills and the ability to collaborate with local providers to ensure continuity of care for

Qualifications

• Bachelor's degree in social work, nursing, psychology, mental health, human services, or related field.
• Master's degree in a related field preferred.
• Experience in behavioral health or related field.
• Must be able to respond to community crises and apply CPR and CPI de-escalation techniques as needed.

Responsibilities

• Engage clients who frequently interact with emergency services in the catchment area.
• Provide client assistance in accessing mental health and medical services.
• Assist clients with accessing public services including financial assistance, housing, and basic needs for food, shelter, and clothing.
• Aid clients with accessing social, educational, vocational, health, and housing resources identified as needs for independent functioning.
• Provide training, coaching, and support in daily living skills including housekeeping, cooking, personal grooming, accessing transportation, budgeting, paying bills, and maintaining independent residence.
• Provide adequate follow-up with clients regarding appointments, completion and submission of forms, and other necessary actions.
• Assist with coordination of hospital discharges as needed to ensure smooth transitions of care and prevent readmissions; engage existing providers to ensure continuity of care.
• Establish and maintain positive working relationships with local community resources including law enforcement, ambulance personnel, crisis call centers, and hospitals.
• Attend agency and community meetings as assigned by supervisor.
• Assist with daytime mobile crisis response as needed.
• Participate in crisis on-call secondary duties as scheduled.
• Engage with clients in a trauma-informed and collaborative manner that supports safety, empowerment, voice, and choice.
• Respond to community crises in a timely manner and apply CPR and Crisis Prevention Institute (CPI) de-escalation and behavioural management techniques as needed.
• Document all client interactions and contacts according to agency and DMH standards.
• Meet all internal and external reporting requirements including data collection for Quality Assurance evaluation and DMH reporting.
• Attend educational seminars and professional development meetings as required.
• Perform other duties as requested by supervisor.
