Qualifications

Responsibilities

• Assessment: admission and ongoing assessment of patient condition; provide holistic nursing care and notify physician of changes.
• Planning: design and document care plans, educate patient/family, plan transitions and safe discharge.
• Implementation: prepare patients for treatments, maintain patient safety, report changes to physicians.
• Evaluation: reassess after interventions and adjust care as needed.
• Report Skills: provide and receive Bedside Shift Reports with family involvement.
