Job summary

Newton-Wellesley Hospital seeks a Registered Nurse to deliver high-level nursing care in the Emergency Department with a focus on palliative pathways. The RN will assess, plan, implement, and evaluate care, coordinating with a multidisciplinary team to ensure patients enter the appropriate palliative care pathways.

The role requires 2+ years of Palliative Care or Hospice experience, RN licensure, and preferably CHPN certification.

Qualifications

• RN with state license in good standing
• CHPN certification preferred
• Minimum 2 years on a Palliative Care or Hospice team within 5 years

Responsibilities

• Identify ED patients who may need Palliative Care consultation
• Perform initial patient assessments in the ED
• Coordinate with ED, Palliative Care, and other teams
• Educate staff on pathway criteria and patient entry
• Track patient progression through care pathways and disposition
