Job summary

Newton-Wellesley Hospital in Newton, MA seeks an experienced registered nurse to provide high-level, population-specific care within a Palliative Care program. You will perform initial ED assessments, participate in cross-disciplinary huddles, and educate teams on pathway criteria to ensure proper patient entry and progression.

Requirements include an active RN license, a BSN preferred, and 2+ years in a Palliative Care or Hospice setting, with ED/ICU exposure.

Qualifications

• RN license in Massachusetts or eligible in MA.
• At least 2 years as RN with Palliative Care or Hospice in past 5 years.
• Experience in Emergency Department and Intensive Care Unit preferred.
• Familiarity with Serious Illness Conversation Guide or Vital Talk.

Responsibilities

• Identify ED patients who may need Palliative Care consultation.
• Perform initial ED assessments including RN Serious Illness Conversation and symptom evaluation.
• Huddle with ED, Palliative Care and other teams to assess pathway suitability.
• Educate clinicians on pathway criteria and entry into pathways.
• Follow patient progression and coordinate disposition with the care team.
• Maintain data and medical records for the ED-embedded Palliative Care Program.
