Job summary

Saint Francis Hospital and Medical Center in Connecticut is seeking a Nurse Senior – Navigator (Complex Care Coordinator) to support high-risk patients in the Emergency Department. The RN collaborates with ED teams to create coordinated care plans and ensures safe transitions across the continuum.

The role requires an RN with CT licensure, BSN preferred, and at least 3 years in ED or acute inpatient care. Part-time, day shift, 24 hours per week, with prorated compensation.

Qualifications

• Graduation from an accredited nursing program, with a Bachelor of Science in Nursing (BSN) strongly preferred and generally expected for successful candidates.
• Valid RN license in Connecticut.
• The ideal candidate will have a minimum of three (3) years of Emergency Department and/or acute inpatient nursing experience, with demonstrated ability to manage complex, high-risk patient populations.

Responsibilities

• Conduct comprehensive, real-time assessments of patients’ medical, psychosocial, and behavioral health needs upon presentation to the Emergency Department.
• Collaborate closely with Emergency Department physicians, advanced practice providers, nursing staff, case management, and ancillary teams to support individualized care planning for complex and high-risk patients.
• Coordinate referrals and facilitate connections to appropriate inpatient services, specialty providers, community resources, and support programs to promote safe dispositions and effective transitions of care.
