Saint Francis Hospital and Medical Center in Hartford, CT, seeks a Nurse Senior – Navigator (Complex Care Coordinator) to support high‑risk patients in the Emergency Department. This part‑time role partners with the ED team to ensure coordinated, patient‑centered care and safe transitions.
The ideal candidate will have a BSN or higher, a CT RN license, and at least three years of ED or acute care experience, with emphasis on care coordination and case management.
In the Emergency Department (ED) at Saint Francis Hospital and Medical Center, the Nurse Senior – Navigator, informally known as the Complex Care Coordinator, plays a crucial role in supporting emergent and high-risk patients with complex medical, social, and/or behavioral health needs. The Registered Nurse (RN) in this role collaborates closely with the ED care team to deliver coordinated, patient-centered care and support safe, effective transitions across the continuum.
