Job summary

Saint Francis Hospital and Medical Center in Hartford, part of Trinity Health, seeks a Nurse Senior – Navigator to support high-risk patients in the Emergency Department. This Complex Care Coordinator collaborates with the ED team to plan care and ensure safe transitions across care settings.

The role is part-time (24 hours/week) with day shifts. Requirements include CT RN license, 3+ years ED/inpatient nursing, and BLS/CPR certification.

Qualifications

• RN license in CT; BSN strongly preferred
• Minimum 3 years ED and/or acute inpatient nursing experience
• Experience with complex/high-risk patient populations beneficial

Responsibilities

• Conduct comprehensive real-time assessments of patients in the ED
• Collaborate with ED physicians, APPs, nursing, case management and ancillary teams for individualized care planning
• Coordinate referrals and connections to inpatient services, specialists, and community resources to ensure safe transitions of care
