Job summary

Episcopal Health Services, Inc. in New York is seeking a Patient Navigator to support emergency department treat-and-release patients who lack adequate linkage to primary care.

Responsibilities include coordinating referrals, guiding patients to follow-up care within 30 days, addressing barriers, and documenting activities in the electronic medical record. Requires strong telephonic and interpersonal communication and teamwork.

Qualifications

  • Minimum 2 years of health experience in a professional setting, working with psychiatric populations.
  • Knowledge of patient navigation, appointment and referral coordination, and healthcare terminology.
  • Knowledge of HIPAA and Privacy requirements.
  • Understanding of Social Determinants of Health, health equity, and cultural awareness.
  • Clear telephonic and interpersonal communication skills and ability to work in a team.

Responsibilities

  • Identify or receive referrals for treat-and-release patients with limited primary care linkage.
  • Interview patients to assess primary care engagement and understanding of first-contact care.
  • Identify barriers to primary care including insurance, transportation, and literacy.
  • Assist with Medicaid eligibility and insurance referrals.
  • Ensure discharge instructions and follow-up obligations are understood; provide education and empathy.
  • Connect patients with health and social services using public and institutional directories.
  • Schedule primary care or subspecialty follow-up within prescribed timeframes; confirm logistics and coverage.
  • Document appointment details in the EMR and discharge paperwork.
  • Follow up within 48 hours after ED discharge to reinforce care plans and adherence.
  • Maintain active follow-up worklists and data for review with leadership.
  • Perform duties in accordance with policies and EHS ICARE values.
