Qualifications

• Bachelor’s degree in the field of human services required.
• 1-3 years’ experience working with programs treating mental health, substance abuse, or co-occurring disorders preferred.
• Experience using electronic health record documentation system/s to document program notes preferred.
• Working knowledge and proficiency with Microsoft Office Suite (Word, Excel, Outlook) and internet search engines.
• Excellent organization and time management skills.
• Crisis Prevention Intervention Training within 1 month of hire.
• Current BLS certification.

Responsibilities

• Assist Psychiatric Providers in coordinating timely treatment and disposition planning.
• Respond quickly to minimize ED or unit overcrowding and boarding.
• Take behavioral health history and assess current functioning factors.
• Identify involved providers and state agencies and HRSN needs.
• Coordinate admissions, bed searches, and transfers to inpatient or partial hospital care.
• Arrange transportation and updates to patients and families.
• Maintain crisis plans and share information with providers as appropriate.
• Provide psycho-education and resources for recovery and support services.
• Complete encounter documentation and maintain resource lists.
