Job summary

DCH Health System is seeking a compassionate patient care advocate to support Emergency Department psychiatric and medically ill patients with behavioral health needs. You will collaborate with ED staff, physicians, and RN/LPN teams to ensure safe admission or discharge plans and to maintain a safe environment.

Daily rounds, vital signs, and ADL assistance are core duties. Responsibilities include monitoring, documentation, patient bathing, health assessments, and adherence to safety protocols

Qualifications

• Approved previous experience in giving patient care or completion of in-service education classes.
• High school education or equivalent.
• Must be able to read, write, speak, and comprehend English.
• BLS required or achieved within first 90 days of employment.

Responsibilities

• Receive RN report at start of shift.
• Walking rounds and shift report with off going PCA.
• Apply cardiac monitoring; change batteries at 7am.
• Document patient bedside rounds q2h.
• Conduct 4 P's on every round: Pain, Position, Potty, Perimeter.
• Document patient vital signs q4h.
• Daily Glucometer QC (Night shift).
• Bathe patients daily; change for central lines.
• Replace EKG pads daily with baths.
• Daily linen change.
• Assist patient with ADLs (brushing, mouth care, feeding, toileting).
• Obtain blood sugars on all diabetic patients and document.
• Stock isolation caddies for next shift.
• Keep rooms clean and free from clutter.
• Empty trash prior to shift changes.
• Answer call lights promptly.
• Initiate or continue pressure ulcer bundle.
• Turn/Reposition patients every 2 hours.
• Fall prevention protocol.
• Weigh all patients on admission and weekly.
• Prepare rooms for admission and update whiteboards daily.
