Ambassador Healthcare in Centerville, IN is seeking a Physical Therapist (PT) for full-time and PRN positions to join our new in-house therapy team. You will perform initial evaluations, develop personalized plans, and deliver skilled PT services to restore mobility and independence.

Collaboration with nurses, therapists, and physicians is essential to ensure seamless patient care. Qualifications include a state license, graduation from an accredited PT program (DPT/MPT/BSPT), CPR certification,

• Graduate of an accredited Physical Therapy program (DPT, MPT, or BSPT).
• Current State Physical Therapist license in good standing (or eligible).
• Current CPR certification (or willingness to obtain upon hire).
• Strong clinical, organizational, and interpersonal skills.
• Previous experience in a skilled nursing or long-term care setting is preferred but not required.

Responsabilidades

• Perform initial physical therapy evaluations and create personalized treatment plans based on patient goals and needs.
• Deliver physical therapy services to restore mobility, reduce pain, and improve physical function and independence.
• Collaborate with an interdisciplinary care team including nurses, occupational therapists, speech therapists, and physicians.
• Educate patients and families on home exercise programs, safety strategies, and wellness practices.
• Supervise Physical Therapist Assistants (PTAs) and ensure adherence to treatment plans.
• Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
• Participate in care planning, team meetings, and quality improvement initiatives as needed.
