Job summary

Sagent Behavioral Health is seeking a School Based Therapist to support students in Woodbury and Cottage Grove, MN in an in-person, 1.0 FTE role. The position focuses on assessment, diagnosis, treatment, and ongoing support within elementary settings.

Responsibilities include collaboration with school staff, parents, and external agencies to foster student well-being and academic success, maintain treatment plans, and provide excellent service within a HOPE-values driven culture.

Qualifications

• Master’s degree in behavioral sciences or related field.
• Candidates will have clinical licensure or be on licensure track for MN: LMFT, LPCC, LICSW, LP.
• Minimum of one year of experience working with children and adolescents.

Responsibilities

• Evaluate mental health diagnoses, create, implement and maintain treatment plans and complete ongoing documentation.
• Coordinate services with case managers, families, work personnel, medical personnel, other Sagent staff, and school staff as needed.
• Provide consultation to parents and school staff.
• Attend school meetings, IEP meetings, Student Intervention teams, etc.
• Attend and participate in all clinical staff meetings and trainings.
• Provide excellent customer service for clients and collaborate with a dynamic team.
